Welcome Letter

Welcome to our practice!  We look forward to meeting you and will do our best to make you comfortable and provide the very best service possible. We believe you will find our doctors to be superior retina specialists, and our staff professional and friendly.

Your new patient forms should be completed prior to your visit if possible.  Bring the completed forms with you to your appointment.  This will save time for you and the office staff, so that you may be seen promptly.

Your examination requires dilation of the pupils, which blurs your vision and may last several hours.  Therefore it is best to have someone who is able to drive accompany you on your visit.  We will provide dark glasses after your appointment to reduce glare from sunlight, and you may wish to bring your own sunglasses as well.

Depending on the nature of your condition your examination and diagnostic procedures may take anywhere from 30 minutes to 2 hours.  Every effort will be made to complete your procedures in one visit if possible.

Ample parking is available at each of our locations.  Parking is free at our Oceanside office. There is a charge of $3.00 (with validation) at the La Jolla location. There is no charge with a parking placard for the disabled.

We accept many insurance plans; however, each patient must take responsibility for knowing the terms of their coverage.  If prior authorization or referral is necessary in your plan, please be sure it has been arranged in advance of your visit. Co-pays must be paid in full at the time of the appointment or a $10.00 billing fee will be incurred. We accept MasterCard, American Express, Visa and Discover credit cards in addition to cash and personal checks. Uninsured patients must make financial arrangements prior to the visit. Nursing home residents must be accompanied by an attendant throughout the visit, and may be responsible for additional fees due to Medicare billing regulations.  You may be asked to provide a photo ID at your first visit.

If you are unable to keep your appointment, please provide us with 24 hours notice.

Please let us know if you have any questions or concerns.  Thank you for choosing San Diego Vitreoretinal Associates for your retinal eye care services.
